Anyone have success mounting a Microdot AND an optic off the upper HG for the VZ either with a NEAT or BS?  Because of their designs I don't think a decent solution exists like the thorntail because unlike the Ultimak, the pic rail end is interrupted and the short stroke give you a short upper. 

Right now I can run a solution off a pic mount slaved to the lower, or a barrel (AK) solution, but I'd rather use the upper.

I have found a way to dremel a Magpul flashight mount, but find it unsatisfactory.  Thanks.

You're asking for a light mount or two optic mounts?

For more rail space both FAB makes or made railed handguards that cantilevered over the gas block.  Should be sufficient for scout scope or other dual needs.

For lights, I mount mine off the lower HG side rail and for AKs and Vzs.  If I'm upgrading, my standard is generally this arisaka mount on picatinny: https://arisakadefense.com/side-scout-mount-picatinny/ 
I like my lights on trigger hand side between 12 and 3 oclock -- keeps them from banging me in knees and junk when slung and helps protect them in most use cases...

But yes, thorntail offset should work given that upper rail extends beyond lower (if you mount all the way to front), but might occlude a bit of your sight picture -- less than 15 or 20% I'd think w/ standard micro red dot, and much of which is prob already occluded by front sight. 
I don't own thorntail's primarily b/c all that modularity introduces add'l potential points of failure, and on the Vz58 threadlockers that are aluminum-safe and heat don't necessarily go together all that well.  Side rail mounts also don't get quite as hot as uppers.

Others should work as well, but will likely be taller than thorntail.  This isn't a use-case I've researched, so there might be thinner steel options at there, but unlikely to be any significantly thinner aluminum ones like thorntail...

All FAB stuff I’ve seen besides latest plastic stuff precludes irons and negates their utility.

I am running a rail piece on the right side with a olight or tlr 1hl pistol light and while it’s easy to use but may not prove robust enough.

I may end up with an RSR BM 1 out on the barrel but would still like to use the rail.  Of course sticking to a rear optic makes the discussion completely different but like the scout position.
