GO to this site www.atf.treas.gov/dcof/index.htm (http://www.atf.treas.gov/dcof/index.htm) and on the right side of page order item #1 then scoll down to f7cr and click it and then click add and then fill out rest of you info and scoll down to submit. Then sit back and wait for you app. in the mail.

Or you can simply call the local BATF office and ask them to mail the application form to you.
