My guns get the most use either shooting USPSA, IDPA and practicing..and found these light modifications help take care of a couple of possible trouble points so I thought I would just post this up, since the question arises every now and then.
couple of light mods to help minimize the top round in the mag walking forward and causing the mag not to drop also install the wolffe +5% or 10% magsprings to help hold the bullet in place. And to help minimize premature slide lock.
Here are a couple of photos that may help..

polish the part of the slide that rides on top of the round in the mag..
also break the bottom edge of the breechface ( see the highlight in the photo ) to minimize any edge that may grab the round in the mag.

stock slide release/lock and a modifiy one. Shortened to avoid contact with long bullets to minimize premature slide lock.

Slide stop is a definite, the breach face can be broken a LOT more then that also polish the rail, that also helps.

i just got my 75BD police back from CZ-USA for warranty repair (ejection probs, pushing the top round forward in the mag). matt, CZ's gunsmith who worked on my pistol put a very aggressive concave bevel where the bar meets the breech face and polished the entire area.
i haven't made it to the range yet, but matt said he test fired it extensively while dropping the mag between shots and pretty much cured the problem.
he also polished the chamber, added an XP extractor spring and a heavier recoil spring (16# wolff) to cure the extraction problems.

Just test fired my new SP-01 today and had the same problem, thanks for the tips...... Another problem I found was that the top of the mag release spring was catching on the nose of round as I dropped the mag also causing it to hang up in the well..... By moving the bend closer to the frame I was able to solve this problem as well.
*NOTE: This only occured when the top round had moved forward in the magazine.

Red is the area where you will polish and break the edge into the breechface.
Yellow is the area to polish that rest on the top round.
