You're asking for a light mount or two optic mounts?
For more rail space both FAB makes or made railed handguards that cantilevered over the gas block. Should be sufficient for scout scope or other dual needs.
For lights, I mount mine off the lower HG side rail and for AKs and Vzs. If I'm upgrading, my standard is generally this arisaka mount on picatinny:

But yes, thorntail offset should work given that upper rail extends beyond lower (if you mount all the way to front), but might occlude a bit of your sight picture -- less than 15 or 20% I'd think w/ standard micro red dot, and much of which is prob already occluded by front sight.
I don't own thorntail's primarily b/c all that modularity introduces add'l potential points of failure, and on the Vz58 threadlockers that are aluminum-safe and heat don't necessarily go together all that well. Side rail mounts also don't get quite as hot as uppers.
Others should work as well, but will likely be taller than thorntail. This isn't a use-case I've researched, so there might be thinner steel options at there, but unlikely to be any significantly thinner aluminum ones like thorntail...
