Current update
Changes made:
Factory recoil spring, extra power extractor spring, lok grips, thin buffer.
I had no gunk when I took the extractor out but cleaned well anyway.
400rd and have no issues since.

Thanks for the update.  I think it was the extractor spring. I have the extra power extractor spring in two of my 75's.

if your extractor is intact & undamaged I don't see how a different type to extractor will help
I think its important to understand what modification were made (if any)

is your extractor hook intact?

I bought a 2nd or 3rd hand 9mm upper for a P-09 and it lost the bottom 1/3 of the extractor hook. That's what can happen when people drop a round in the chamber directly then drop the slide. (I never did it, but it is likely the first owner did)
