It looks like the optic plate is defective and bowed. It looks like there might have been something under the front of the plate preventing it from fitting flush and the screws were tightened down so tight it bowed the plate, but I'm surprised the threads didn't strip out before bending the plate. Either way, the operator who installed the plate should have easily noticed that and quality control should have picked it up.
I don't think that simply over torqueing the screws could cause the plate to bow like that unless there is something under the front of the plate. The cut for the optic looks perfectly straight, it's just the cover that is bent.
