I have had two squibs in the 10+ years hand loading, both on a four hole Lee turret.  I surmise that when I would pull a charged case out to check the weight of the powder thrown that the auto index would advance the case which lead me to seat the bullet on the now empty case.  When the plastic ratchet failed, instead of replacing it, I have since advanced the dies by hand...when I am ready to do so.  I haven't had a squib since and that is in many years.

What you say in this post does not make any sense.  You say when you pulled the two cases out to check the powder thrown some how the turret "automatically" advanced to the bullet seating stage? The Lee 4 hole turret only advances when you physically pull the lever, not some how "automatically" advance. I say this because you say the plastic ratchet failed and you never replaced it and turn the turret by hand (manually now.

Second, if you pulled the case to check the powder how did an empty case get back into the shell holder, if you were checking the weight of the powder, did pour the powder back into the case and put that charged case back into the shell holder after weighing?  If you left the powder in the pan on the scale then you must have put that now empty case back in the shell holder on the press. But again the turret will not move while you are doing this...you have to pull the lever or you pulled the lever after dropping the powder and turning the turret to bullet seating die.

Third, if you had looked into the case and checked the powder before you seat a bullet you might have caught those two.

IMO you surmised wrong in recognizing your issue...it was not the press....there is an old saying "It's not the weapon, it's the warrior"

Take what I'm saying with a grain of salt....I have loaded for years on a Lee Classic Turret and how you describe this just simply can't happen without human error.
