Went to the range today for some live fire, sight check before this weekend's match. Loaded mags, sent the target downrange, double action pull feels much smoother after some polishing and grease, and...
Soft thud, FTF. Racked slide and caught the round - no primer strike. Tried again, same result.
Black O-ring was well camouflaged in the black hammer groove. Removed the O-ring, fires much better now. D'Oh!

Sorry that it happened to you. That’s the reason why I like the orange o-rings better. It’s easier to see them compared to the black ones.

I have also done that at the range, with the black o-rings I picked up at Home Depot.  That's why I then ordered some red ones off of some internet site.

I picked mine up at HD too.  I wish there was a way to keep them in the channel a little bit better.  Manually cocking the hammer a few times causes the O-ring to fall out.

Did you buy this o-ring? https://www.homedepot.com/p/Hillman-3-8-in-O-D-x-1-4-in-I-D-x-1-16-in-Thickness-Neoprene-O-Ring-12-Pack-780007/204726333
I only see one with 3/8 OD & costs almost $6

Look for DANCO #83 o-ring.  They are below $3 per 10 pcs.

o rings are more expensive and do not stay in place as well as a piece of vacumn hose the correct diameter that is sliced with a razor blade to whatever thickness works best for you. been using it for years and if i new how to post pics here. use to use photo buddy! i would post one. even bright yellow gas line hose for small engines would work! and the yellow would be more visible!

The Polk County resolution includes  a clause
to the pro-resolution stance. It says,
“The criminal misuse of firearms is due to the fact that criminals do not obey laws, and this is not a reason to abrogate or abridge the unalienable, constitutionally guaranteed rights of law-abiding citizens."
