LPA sight issues/questions

Hello all! Been having some issues with my 85 combat's adjustable rear sight and had some questions. Bought the pistol new back in December, I've since put somewhere around 4000 rounds thru it since then. Great gun, blast to shoot, reliable, accurate, good looker, exactly what I want in a full size all metal 9mm. However back in March it started shooting all over the place, that's when I noticed the rear sight was about to fall out of the dovetail. The gunsmith at my local range was able to center it properly back in the dovetail and give it a quick stake to keep it tight in the dovetail. Dialed the sight back in and was shooting tight groups at 7 and 10 yards. Fast forward to the end of May, I'm shooting all over the place again, even at 5 yards POA/POI is way off. This is happening with multiple brands of ammo. Sight hasn't moved in dovetail at all and I haven't touched the adjustments since the gun was fixed originally and dialed back in. I shoot my s&w .38spl revolver or my ar-15 and I can shoot great groups at 10 yards pistol and 25 yards rifle (indoor 25 yard range) so I doubt its my shooting skills. What gives?

Has anyone else had issues with the rear sight coming loose or losing adjustment over time? Also, I'd love to have night sights as I can't see the dot's that well on the LPA sight, anyone install the meprolight night sights from CGW on their 85?

Check the sight blade for any looseness. either side to side or up and down.
adjustable sights have lots of little parts, so they can break.

There is definitely looseness in the sight blade going from side to side, enough to make me uncomfortable. The adjustment screw for the height seems to wiggle around when I move the blade side to side. As for up and down, there is no looseness going up with the blade, but pushing down there is. However pushing it down feels like its spring loaded, as it pops right back up in place when I remove my finger. I can push it all the way down until the blade touches the slide itself.

Is it safe to say this sight is probably broken?

I would say something is up with windage. elevation sounds fine.

I agree. Personally I would like to get rid of the adjustable sight.

Stuart, does CZ custom offer a matching tritium front sight post for H-TAC tritium rear sight?

yes..for your pistol you would pick the 6.5 tritium front sight.
