I had my form 1 approved and took it to the local engraver who I've always used without ANY issues at all in the past. He has done 5 for me in the past and all of them look perfect, until he did the Scorpion.
Everything is spelled correctly and to ATF specs, but it was not laser engraved in the location that I wanted. I told him specifically and even left him with a picture showing my trust name and location directly below the spot on where it says CZ USA, KANSAS CITY, KS in one line to match the factory engraving.
Instead, he engraved it to the left of the ejection port with my trust name and location on two separate lines. Needless to say I am not happy and did not pay for this engraving. Now, I am either stuck with this or hoping I can find someone that can fix this somehow. I have no idea. I haven't called CZ and doubt they would allow me to buy another plate wth matching serial number to match mine but maybe it's worth a shot.
