Sbr form question for those that have been approved

This is my first sbr form and I'm a little nervous and don't want to mess up. I have a trust set up already. Just not sure about the company name and gun name. All help would be appreciated.

I used "CZ SCORPION EVO 3 S1" for the model on my form 1, if you are using a trust you need to check the box where it says "Corporation or Other Business Entity", and put the name of your trust and address. Also put your trust name in the box where it says "Trade name if any". You do not put your name on the form except where you sign the form, you sign it as a trustee. This is how i filled my form out and it took about 4 months to get approved with no problems. Oh also put "CZ-USA, UNITED STATES" for the name and location of the original manufacturer. The hardest part is the waiting game. Good luck!

I used "CZ (CESKA ZBROJOVKA), CZECH REPUBLIC", and I agree, CZ USA would probably be rejected as they are the importer not the manufacturer.

I also have seen where some people were approved with "Scorpion EVO 3 S1", but recently the BATFE folks have rejected that and require "CZ Scorpion EVO 3 S1"

On eForms I didn't even see CZ-USA as an option, maybe I missed that one? It is almost entirely foreign parts, and is an imported weapon I thought. I was also told to leave trade name empty since the applicant name will be the name of your trust

That's how I filed my form 1 and had no problems with it maybe things have changed or it depends on who is processing your form.

Thanks for all of the input.  My efile is done and the wait begins. After waiting 6 months for my silencer this should be a breeze.
