Something really hard got in there like a shard of spring or that sort of thing. If you are comfortable with disassembly, inspect your magazines and frame for defects. The slide can be cleaned up with a file and a little polish. Otherwise it should be sent back to CZ for inspection and repairs.

It's not the lifter spring, check.

Is this a safety or decocker?

Is the ejector all there?   How about the slide stop retention spring?  If a safety model, are all the safety detents and springs present?  How about the trigger bar spring?

The slide damage didn't cause the frame damage but I agree that something might have been bouncing around in there.

Time to do a detailed inspection on all the parts.

I would send it back to CZ for repairs / evaluation. That's some serious peening. Something hard got loose in there. It may be recoil spring related in some way or other. Generally the outer spring should be changed every 500 rounds or so.

How far did the pistol eject shells when fired?

I meant to pay attention to that today, but totally forgot to. It did handle 50 rnds of 124 S&B and 10 rnds of federal 150 gen HST micros without a hiccup, but still not sure I want to invest in doing the upgrades and put her back in rotation until she's cleared by HQ.
