The extractor retains the firing pin. Did you try replacing both the firing pin and extractor with a new part?

From the looks of your firing pin pic, the forward edge of the retaining slot looks to be beveled/angeled forward rather than straight perpendicular, which could allow it to move past the extractor -- I would think that in doing so the extractor would get out of alignment with the rest of the bolt and force bolt in opposite direction (would also make sense to replace extractor spring and might as well do the retaining piece as well). So on rear of bolt (assuming that's what you're referencing by "breech block," that's probably from impacting your recoil spring retaining rod -- the part attached to the top cover -- as the bolt was forced down and to the left. Check top cover/spring guides for damage.
Are you sure you're not having out of battery, chamber over-pressure, or other issues? Or at least something like that didn't cause in the first place?
And I'm frankly at a loss for what's going on with your striker's front face.
