Medium for me but i also use a grip rap for the added palm swell. Here is a pic with the Kadet slide installed.

Will work for CZ pics! (including but not limited to all CZ clones)

I wear a men's large in gloves so my hands are big but not huge.  I'm 6' 3" and weigh about 210 but I like the small back strap.  I've tried the others several times on both my P-09 and my P-07's but I keep going back to the small.

I like the grip a little more slender.

For me it?s medium. I wear large gloves, but am happy with the medium back strap. Agree with TOK36 that a palm swell would be more helpful for me than the large backstrap

Medium.

Which one should you be using?

For me it's the one that doesn't mess up my grip/sight alignment.

If I have the right grip panel in the pistol frame - or if the pistol fits my hand right (CZ85/P01/CZ75 Compact) as well as my XDM and M&P and 1911 handguns - also use the medium grip insert in the M&P FS pistols, can't remember about the XDMs, that was years ago) then the sights are aligned when I bring the handgun up to aim/shoot whether the target is in front at shoulder height - or down on the ground off to the left/right, or up high off to the left/right.

Never had such an issue till I decided to play with the inserts in the M&P's and see which ones felt best in my hand.  It was the large insert.  Pulled out the medium inserts in both pistols and installed the large.  Felt good.  I was happier.  One day, while walking the dogs I decided to do some dry firing, so I unloaded the M&P 9MM and put it in the holster.  Pulled it from the holster, aimed at the light on the big trailer down near the wood line and it was "good" - click, target hit, if I'd been firing.

Racked the slide put it back in the holster and then pulled it out and aimed off to the left at spot of yellow leaves in a brush pile.....WHERE's THE FRONT SIGHT????  I had to wiggle the pistol around with my wrist to find the front sight and line it up.  Weird.  Racked the slide and repeated it.  Same thing.  Had to "find" the front sight.  Racked the slide and aimed off to the right.  Same thing.  Low, high, high left/right, low left/right.  Anywhere but directly in front of me and the front sight did not come up lined up with the back sight.

Sure, i could have kept at it till my muscles/eyes/mind learned how to have my wrist angled correctly regardless of where the target was - but, then what would my other pistols be "doing" when I dry fired them high left/right or low left/right?  I figured then the other pistols would be "off" so I put the medium inserts back in the M&P's and left them alone.

Try it.  See if the insert makes a difference in your pull from the holster and aim off at an angle at a target sight picture/sight alignment.  If it doesn't, you've go the right sight insert in.  If it does, you make the choice.  "Learn" the new angle/hold and chance things being off with your other handguns or work through the inserts till you find the one that works and doesn't force you to change.
I just keep wasting time and money on other brands trying to find/make one shoot like my P07 and P09.  What is wrong with me?

Same here.  I wear XL gloves and went back and forth between the Medium and the Large before deciding that the Large felt best and my P-09 pointed better for me with the Large.

I only have medium hands but definitely prefer the large back strap.  It makes the shape of my strong hand easier to grab firmly with my weak hand, and also gives me a little more room for the meaty part of my weak hand thumb/palm.

I'm the same. I have grown to like a bigger size grip. I don't have huge hands, but I really like the size of my Gen4 Glock 21.

Medium hands, but love the feel of the small backstrap.

I'm going to hijack* this thread because while I was trying to figure out my backstrap preference I noticed that my grip "squeaks" whenever I grab the gun. Its that unmist noise you get when two pieces of plastic are being tighrlt forced together. It's not loud but it also comes with a settling movement. Like a very slight 'click' as the backstrap shifts into the proper position.

Does anybody else have this? It might not seem like a big deal but it's driving me crazy. I absolutely love the gun otherwise but I've never had any poly gun do this before.

Although I like the feel of the small, I shoot better with the medium. I find the best backstrap is the one that, when looking top-down at the gun, allows for the straightest pull of the trigger backwards. All the joints in your fingers align differently with different backstraps, and the medium allows my fingers to align in such a way that the tigger pull is straight back. Small and large tend to have me pulling the gun left or right. I suggest going with what produces best mechanics (ergo results) instead of best feel.
