The VZ58 "gremlin" is a condition that can result in the striker not being caught by the sear and follwing the carrier home, leaving the rifle uncocked.
The "gremlin" shows itself when for one of several reasons, the sear re-connects to the disconnector after the carrier has disconnected them during its rearward travel.

He follows that with a pretty detailed explanation of how the re-connect actually occurs, and why a tabbed bolt carrier solves the problem in semi-auto VZs

I TIG weld them into place, get them real nice and hot, then quench them in oil.  I charge $50 shipped if you send me your old bolt carrier. 

I've tabbed many a bolt carrier.
